Austin Gastroenterology now offers Chronic Care Management (CCM), a specialized service for patients with two or more long-term health conditions. This program is designed for individuals with health issues expected to last at least 12 months that put them at risk of functional decline.

Our program is designed to provide continuous, remote support to help you manage your health between regular appointments. We'll work with you to ensure you stay on track with your health goals, giving you peace of mind and extending your care beyond our clinic's walls.
