What to Expect

We understand you may be nervous before your first visit to a gastrointestinal specialist and may not know what to expect during a gastroenterology exam. We put your treatment, comfort, and convenience first, and it shows when we get to know you, your symptoms, and needs.

We have put together quick lists of documents to bring to your AG appointments. This information usually depends on the type of appointment and whether you've been to AG before.

Initial Consultation

If it is your first time at AG and you are visiting for a screening or consultation, we kindly ask that you please bring:

  • A government-issued photo ID
  • Proof of address*, if different from your photo ID address
  • Your medical insurance card or other applicable information
  • Patient Information Sheet
  • General Consent
  • Financial Policy
  • Initial Patient History and Intake Form

*Austin Gastroenterology accepts recent utility bills, bank statements, or other official documents that list your current address for proof of address.

Treatments and Follow-Up Visits

If you are coming in for treatment or a follow-up visit, we kindly ask that you please bring:

  • A government-issued photo ID
  • Proof of address*, if different from your photo ID address
  • Your medical insurance card or other applicable information
  • Patient Information Sheet (This form must be completed once per year for every patient)
  • Other forms, based on your procedure**

*Austin Gastroenterology accepts recent utility bills, bank statements, or other official documents that list your current address for proof of address.
**Please consult with your physician’s office for the exact forms needed for your follow-up visits.

Procedures

For procedures requiring anesthesia, you must have a responsible adult stay with you on the day of your procedure and take you home post-procedure. Please note, non-medical ride services, including taxis, Uber, and Lyft, are not acceptable forms of transportation for your procedure. If you do not have someone to drive you, Austin Gastro has access to several approved and reliable transportation services. These approved transportation companies are:
