We respect your time and we want your visit to be processed as efficiently as possible. You can help by remembering to bring the following items, as required, to our physicians' offices or to the endoscopy centers:

• a complete list of all medications, including over the counter medications.
• copies of medical records (labs, ultrasound/CT reports, etc)
• your Insurance ID card
• your Driver’s License
• your Referral Authorization if your HMO requires it

Additionally, if you are scheduled for an endoscopic procedure, you will receive a Patient Personal Information/ Patient Health History Form from your doctor's office. Please fill it out and bring the completed form to the Endoscopy Center. This form may also be downloaded from our forms page.
